Proposed fields for alumni list

I'd like a collection of fields that doesn't gather more information than is necessary (brief). It is primarily basic contact info. "Find me on" field will direct us to other places that the person has information (or can be reached). I included LCF roles and IV staff because this information isn't collected anywhere else.
